The City of Racine is committed to the health and well-being of its employees. As part of its benefits package, the City of Racine offers the City Wellness Program to employees, retirees, and their spouses who are insured through City health insurance.
By completing the steps below, you will receive $200 (and your spouse $100) as the City Wellness Incentive. Further reimbursements are available; see below for details.
New: Free 15 minute Blood Pressure and Glucose/Diabetes Screenings, including a consultation, will be offered in March. Register at www.aurora.org/apptregcityofracine.
Note: Biometric screenings are not required for the City Wellness program. However, you may be screened for your own well-being by the Racine County and City Health Center at Wheaton Franciscan Health Care - All Saints, or by your personal physician.
City Wellness Program participants are surveyed annually to determine outcomes of the program. The feedback received from the survey also helps to facilitate continuous program improvement. Below are the results of 2014 City of Racine Wellness Survey.

Contact Debbie Mazius (262-636-9491), the Racine County and City Health Center (262-687-8640), your personal physician, or your Department Physician if an employee of the Fire Department. This is a confidential consultation to assist you in understanding your health assessment and set your personal goals.
After the consultation, have the Wellness Incentive Form signed. Return it along with the Rally Survey completion page (example above) and Wellness Evaluation Form to Human Resources, City Hall, 730 Washington Avenue Room 204, Racine, WI 53403.

In its commitment to employee wellness, the City provides a reimbursement incentive for employees, retirees, and spouses utilizing a nicotine gum. The City's medication card, CVS Caremark, entitles you to prescription smoking cessation medications. See the attachment for more information about this benefit.

The Racine County and City Health Center is a benefit for insured City employees, dependents (>2 years old), and retirees. Staffed by Nurse Practitioners, the Center offers treatment for acute and chronic illnesses (including sore throat, earache, and high blood pressure), giving immunizations, conducting physicals and more.
The Racine City Health Center is located at the Wheaton Franciscan Healthcare - All Saints Spring Street Campus, West Professional Building - A, 3805 Spring St., Suite 210, Racine.

The Employee Assistance Program (EAP) is a program by Wheaton Franciscan Healthcare - All Saints designed to provide professional counseling support to insured City employees for a variety of work and personal matters, including relationship/marital problems, stress, financial pressures, and alcohol/drug abuse issues.
Services are confidential and provided free of charge to you and your dependents. EAP services are provided at a variety of locations throughout the Racine community. Visit the
