Unified Health Improvement Plan
In collaboration with community partners, the Public Health Department has developed the Unified Health Improvement Plan (UHIP) for residents and stakeholders within its jurisdiction.
The UHIP is a comprehensive document that includes the City’s Community Health Assessment, Community Health Improvement Plan, and the Public Health Department’s Strategic Plan. The Public Health Department is requesting that residents read the UHIP and provide feedback. This feedback will be used to shape future health priorities, and contributes to the planning for Public Health Department programs and service offerings.

Community Health Assessment
The Community Health Assessment establishes the overall health status and identifies the top health concerns facing residents in the City of Racine. These were determined through individual telephone surveys done in partnership with Advocate Aurora Health and several other community organizations, and through analysis of the 2018 County Health Rankings.
This section includes demographic information and data related to individual and community health in twelve categories, which correspond to the Focus Areas in the State’s health plan, Healthiest Wisconsin 2020. The Focus Areas include:
- Adequate, Appropriate, and Safe Food and Nutrition
- Alcohol and Other Drug Use
- Chronic Disease Prevention and Management
- Communicable Disease Prevention and Management
- Environmental and Occupational Health
- Healthy Growth and Development
- Injury and Violence
- Mental Health
- Oral Health
- Physical Activity
- Reproductive and Sexual Health
- Tobacco Use and Exposure
Residents who participated in the surveys also ranked their top health concerns. The top five priorities are listed below in order of importance:
- Access to Health Care
- Illegal Drug Use
- Chronic Diseases
- Cancer
- Overweight/Obesity
As part of the Community Health Assessment, key stakeholders/informants in the community were interviewed and asked to identify their top health concerns facing City of Racine residents. The Key Informant priorities are listed below in order of importance:
- Mental Health
- Alcohol and Other Drug Use/Substance Abuse
- Chronic Disease Management
- Access to Health Services
- Nutrition
Community Health Improvement PlanThe information and data resulting from the Community Health Assessment was collected and analyzed in order to establish the Priority Health Areas for the Community Health Improvement Plan. For each of these Priority Health Areas, the Public Health Department designed goals, measurable outcomes, and implementation strategies aimed at addressing selected health concerns. These goals were determined based upon overall priority, overall need, and where the Public Health Department and its partners will be able to make the biggest impact, with the highest budgetary efficiency, to improve the health of the Racine community.

- Chronic, Communicable, and Preventable Disease
- Healthy Homes
- Natural Resource Protection
- Child Lead Poisoning Prevention
Strategic PlanThe final component of the UHIP is the Public Health Department’s Strategic Plan. The Strategic Plan consists of the following five elements specific to the Public Health Department:
- Mission and Vision Statements
- Core Values
- Review of Strengths, Weaknesses, and Opportunities
- Objectives, Strategies, and Operational Tactics
- Metrics for Measuring Success
Management personnel in the Public Health Department compiled, reviewed, analyzed, and prioritized the results of an all-staff exercise ranking of Departmental strengths and areas for improvement. The results encompass short and long term goals and objectives that are feasible, given current staffing, budgetary, and facility constraints. Implementation of these goals and objectives leads to higher staff morale, increased efficiency, and improved response and customer service.
